RULES AND REGULATIONS FOR THE INSPECTION AND/OR COPYING
OF THE PUBLIC RECORDS OF THE INDEPENDENCE FIRE DISTRICT


 
I. Public Records Inspection

Non-exempt public records of the Independence Fire District may be inspected during the
regular office hours of the District (7:00 a.m. to 3:00 p.m.), prevailing time, Monday through
Friday, except for District holidays), provided that the District receives a written application
therefore by hand delivery, mail, or facsimile, which specifically describes the records to be
inspected, and signed by the applicant, with the name of the applicant legibly printed thereon.
If the public record is in active use, in storage or not otherwise available, the official custodian
shall immediately notify the applicant and shall designate a place, time and date for inspection
of the public records, not to exceed three (3) days from the receipt of the application, unless a
detailed explanation of the cause is given for further delay and the place, time and earliest date
on which the public records will be available for inspection is specified.

Independence, Kentucky, 41051.
Please download and submit a completed Open Records Request Form to the Fire Chief of the
Independence Fire District, who serves as the official custodian of the public records. This form
may be found on our website at www.independencekyfire.org.

II. Copying of Public Records

Copies of the public records of the Independence Fire District are available upon the receipt by
the District of a written request therefore, and the advance payment of any required postage
and a fee of $0.10 per page for copies for non-commercial purposes, and a fee for copies for
commercial purposes of the total of both:  (1) the cost to the District of media, mechanical
processing and staff required to produce a copy of the public record or records, and (2) the cost
to the District of the creation, purchase or other acquisition of the record to be copied, as
determined by the District Fire Chief.

your record, please call 888-410-6986.
Attorney Offices & Legal Copy Services
All requests for medical and/or billing records are handled by our biller, Medicount Management.
Medicount fulfills records requests electronically via ChartSwap.
ChartSwap is a HIPAA compliant platform launched to facilitate electronic medical and billing
record exchange between medical providers and law firms, and other requesting parties.  It’s
free to register and requestors can use ChartSwap to request, track, pay for and download
records. 


To submit requests via ChartSwap, use the following steps.

 Register at www.chartswap.com as a Record Requestor;
 Sign in and Search for a Provider, Enter Request Details, then Upload Supporting
Documents;
 Once your request has been reviewed and records are available, you will receive a
notification and invoice, which you can pay with a check or credit or debit card.
